By signing this form, I, the parent or legal guardian of the participant, acknowledge and agree to the following terms and conditions in relation to my child’s participation in Acrotrix activities, including but not limited to workshops, intensives, classes, and use of Acrotrix equipment.

Assumption of Risk & Participation Acknowledgement

I understand that participation in Acrotrix activities involves inherent risks, including the risk of injury. I confirm that I am voluntarily allowing my child to participate, and I acknowledge that Acrotrix and its organisers have taken all reasonable and practical steps to minimise these risks through appropriate safety measures and supervision.

My child will:​

• Follow all safety instructions given by Acrotrix staff.

• Refrain from attempting any skills beyond their current ability level or that could endanger others.

Health & Medical Declaration

I confirm that my child:

• Is between 3 and 18 years of age.

• Is in good health and is not suffering from any medical condition, disability, or injury that may affect their ability to safely participate.

• Is over 1 metre tall (where height is required for participation).

I acknowledge that Acrotrix has not provided medical advice relating to my child’s physical condition or their ability to participate, and I confirm that I have independently assessed that my child is fit to take part.

Liability & Indemnity

I agree to be responsible for any damage caused by my child due to negligent, reckless, or dangerous behaviour.

I fully indemnify Acrotrix, its organisers, staff, and associated companies against:

• Any claims, demands, or liabilities arising from injury to my child or to any third party caused by their participation in Acrotrix activities.

• Any loss or damage to property resulting from my child’s actions.

Photography & Videography Consent

I grant permission for Acrotrix to photograph or record my child during classes or events. I understand that these images or videos may be used for promotional purposes, including social media, websites, and other marketing materials. No financial compensation will be provided for the use of such media.

Legal Agreement

This agreement is governed by the laws of England. If any provision is found to be invalid or unenforceable, the remaining provisions will remain in full force and effect.

I confirm that I have read and understood this waiver and that I am signing it voluntarily on behalf of my child.
